It is important you keep us updated with any personal changes like your name, address, phone numbers or email address, to do this you will need to visit the surgery during our opening hours, continue reading for instructions on what documentation you need to provide.
Change of address
If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.
Changing your phone number or email address
You will need to show us identification before we can change your phone number or email address.
Changing your date of birth or name
You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
Finding your NHS Number
There are many reasons you may need your NHS number for example booking your COVID vaccination or registering for the NHS app. If you are on a repeat medication you can find your NHS number on the repeat side, if you are not on repeat medication use the link Find you NHS number
