Freshney Pelham Primary Care Network (PCN)

The Freshney Pelham Federation PCN is a network of practices in the Northeast Lincolnshire area, formed in October 2018 connecting 3 local GP practices.

Federations are a new development initiated by the Government to develop working across practices, sharing of experienced staff, and allowing flexibility of working, helping to ensure a strong and secure NHS services for the future.

In total, these 3 practices have over 37,000 patients registered.

The Freshney Pelham Federation is made up of:

  • Woodfield Medical Centre
  • Littlefield Surgery
  • Pelham Medical Group

Our practices have come together to form a PCN so that as a federation we can:

  • Support patients on a more local level.
  • Offer more services to improve the patient experience meet the needs of our patients.
  • Support the capacity and reduce pressures in practice.
  • Develop and deliver more efficient services for patients across the area.
  • To generate income which can be re-invested into patient care locally.
  • Improve and enhance the current workforce model.

As a PCN the practices develop and provide new services to improve our patients care, these clinical roles remain under the mentorship and supervision of a General Practitioner at all times. The clinicians actively work towards developing and maintaining effective working relationships both in the practice and the community while developing strong links with locally available services.

  • Paramedic
  • Health and Wellbeing Coaches
  • In-house Counselling Service
  • Social Prescriber
  • Patient Advice Service
  • Care Co-coordinators

Our Care coordinator’s provide administrative support across the the practices and may contact you to book routine annual health checks, chronic disease health checks, immunisations and care reviews.

District Nursing for Freshney Pelham Care

Our District Nurses are highly skilled registered nurses who have undertaken a specialist qualification in community nursing. Each District Nurse team is led by a District Nurse Specialist Practitioner, and includes registered Community Staff Nurses, Assistant Practitioners and community Healthcare Assistants who support the registered nurses with specific aspects of nursing care.

District Nurses visit people in a range of residential settings, who are unable to visit the GP surgery and who require nursing care, advice and support due to being permanently or temporarily house bound.

Care Homes

Our District Nurse team also ensures those patients with chronic and complex conditions receive timely and appropriate support to those in a care home. Led by an experienced District Nurse Specialist Practitioner with advanced skills who addresses the care, coordinates the are required for the nursing needs as the condition improves or declines. The aim is to ensure that the patients care focuses on ‘what matters to me’ ensuring you have a tailored personalised care experience to improve health and quality of life.
