Private Fees: Frequently Asked Questions

Isn’t the NHS supposed to be free?

The NHS covers most healthcare, but not everything. Charges exist for some services, such as prescription fees and insurance reports, which fall outside the NHS contract.

Aren’t GPs already paid for this?

GPs are self‑employed, not NHS employees. The NHS pays for NHS work only. Non‑NHS tasks must cover the GP’s costs—staff, building expenses, utilities, administration and the time required to complete the work.

What is covered by the NHS and what isn’t?

The NHS contract covers medical care for NHS patients. Increasingly, other organisations ask GPs to provide non‑medical documentation simply because they are trusted professionals. These tasks fall outside NHS-funded care.

Why do some letters cost £40 while others cost more?

Costs reflect:

  • The time needed to review your full medical record
  • The complexity and length of the requested letter
  • Dictation, typing, printing, and postage costs

Some requests are straightforward; others require extensive review to ensure accuracy.

Why can it take time to complete my form?

NHS clinical care must always take priority. Paperwork is completed outside routine clinical duties and adds to an already heavy workload.

I only need the doctor’s signature—why is this still chargeable?

A GP must only sign information they know is accurate, which often requires reviewing your entire medical record. Incorrect or careless signing can have serious professional consequences.

How You Can Help

  • Check if a doctor’s signature is truly required.
Some documents (e.g., passport applications) can be signed by other trusted professionals.
  • Use the NHS App.
You can access and print your immunisation history, medications, and medical records—useful for applications such as Blue Badges.
  • Submit multiple forms together.
Your GP may be able to process them together, potentially at a reduced cost.
  • Allow enough time.
Private forms can take up to 4 weeks to complete.
